Position Summary We are seeking a dedicated and compassionate
Physical Therapist
to join our Home Health Care team. In this role, you will deliver one‑on‑one therapeutic services to older adults in their homes, helping them regain mobility, improve function, and maintain independence. This position offers autonomy, flexibility, and the opportunity to make a profound difference in the lives of seniors and their families.
As a nonprofit organization, Mozaic Senior Life is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program.
Responsibilities
Conduct comprehensive evaluations to determine patients' functional needs and develop individualized treatment plans.
Provide evidence‑based physical therapy interventions in the home setting.
Educate patients and caregivers on exercises, safety techniques, and equipment use.
Collaborate with an interdisciplinary team including nurses, OT, SLP, social workers, and physicians.
Document assessments, progress notes, and care plans in accordance with regulatory and organizational standards.
Adjust treatment plans as needed to ensure optimal outcomes and patient satisfaction.
Participate in case conferences, team meetings, and quality improvement initiatives.
Ensure compliance with all home health care regulations and agency policies.
Qualifications
Current Connecticut Physical Therapist (PT) license in good standing.
Doctor of Physical Therapy (DPT) or Bachelor's/Master's in Physical Therapy.
Previous experience in home health, geriatrics, rehab, or related settings preferred.
Strong communication, critical‑thinking, and patient engagement skills.
Ability to work independently and manage a flexible schedule.
Valid driver's license, reliable transportation, and ability to travel within service areas.
