A respected 200+ bed acute care hospital near West Palm Beach, FL is seeking an experienced
Director of Case Management
to provide strategic and operational leadership for a dynamic case management team. This is an excellent opportunity for a proven healthcare leader to make a measurable impact on patient outcomes, care coordination, and hospital performance metrics.
Position Overview The Director of Case Management is responsible for the fiscal, operational, and personnel management of the Case Management Department, including RN Case Managers and licensed social workers. This leader ensures appropriate patient status placement, effective utilization management, and coordinated discharge planning to promote safe transitions of care and reduce readmissions.
The Director will work collaboratively with the Chief Medical Officer, medical staff, and hospital administration to meet length of stay goals, enhance physician engagement, and drive compliance with payer and regulatory requirements.
Key Responsibilities
Lead and manage the Case Management Department, ensuring effective care coordination and patient flow.
Oversee utilization management, ensuring appropriate patient status and compliance with hospital goals.
Coordinate discharge planning processes to ensure safe, timely, and effective patient transitions.
Serve as the hospital lead for the
Utilization Management Committee
and co-chair the
Denials Committee
with the Patient Access Director.
Collaborate with physicians, nursing leadership, and payers to minimize denials and manage post-discharge utilization reviews.
Develop strategies to reduce avoidable readmissions and improve outcomes across the continuum of care.
Manage departmental budgets, staffing, and performance metrics to align with organizational objectives.
Qualifications
Current
Registered Nurse (RN)
license in the State of Florida.
Bachelors degree required ; Masters degree (MA/MS) strongly preferred.
Case Management Certification
required.
Minimum of
5 years of progressive leadership
in Case Management within an
acute care hospital
setting.
Strong understanding of utilization review, discharge planning, and payer relations.
Demonstrated ability to lead interdisciplinary teams and collaborate effectively with medical staff and administration.
