Why M Physicians? The Pediatric Discovery Clinic offers a wide range of services to pediatric patients seeking care from any of our 15 sub‑specialties. Some of the pediatric sub‑specialties we see include surgery, dermatology, endocrine/diabetes, nephrology, urology, gastroenterology, pulmonology, muscular dystrophy, allergy, solid organ transplant, weight management, and adoption medicine. Many of our clinics are multi‑disciplinary, meaning we offer multiple specialty services within one clinic appointment. We perform outpatient procedures, pulmonary function testing, lab draws, allergy testing, and other interventions as directed by physicians within the clinic. We offer child‑family‑life services, which help children to cope and understand procedures in a child‑friendly way. Our teams practice patient and family centered care, partnering with families to ensure they are partners in the care they receive.
What You Will Do As an RN Care Coordinator
Coordinate care across the health care continuum within designated sub‑specialty population(s)
Serve as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and services
Complete pre‑visit planning and post‑visit wrap‑up, determining vital follow‑up appointments and/or treatments to optimize patient visits
Facilitate proactive disease management and preventative health care using evidence‑based practice
Room patients for the surgeon(s) as needed, complete clinic readiness including records review, and provide administrative support as needed
What You Will Need
Current Minnesota RN license
BLS certification
Demonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacy
Clinic Description: Pediatrics Discovery Clinic Hours: 1.0 FTE, 40 hours per week

Compensation $76,211.00 – $121,930.00 USD Annual
