CERTIFIED OCCUPATIONAL THERAPY ASSISTANT/CASE MANAGER

Cabvi is seeking a Certified Occupational Therapy Assistant/Case Manager to provide occupational therapy to children and adult consumers, enabling them to achieve their highest level of functioning. The role involves reaching out to consumers, completing intakes, developing service plans, coordinating services, processing referrals, and providing services to consumers who are blind and visually impaired. Key Responsibilities

Key Job Elements Provide occupational therapy to adults and children under the supervision and clinical direction of an Occupational Therapist. Assist Occupational Therapist in developing occupational therapy protocols, procedures, interventions, and policies. Meet contractual and grant obligations as well as established occupational therapy clinical productivity goals. Assist in the development, planning, coordination, and execution of vision rehabilitation group instruction for community programs, residential programs, staff training events, and external community support events under the direction of a certified VRT or OT. Provide vision therapy training to children identified through screenings and direct referral. Instruct consumers in use of low vision aids and adaptive equipment. Maintain awareness of current academic material and research; continue professional development in the rehabilitation field. Assist preschool vision screening team with completing screens, organizing paperwork, and processing results. Participate in training, conduct research, and collect data related to new initiatives and grants within the department. Respond to consumer inquiries and reach out to inactive consumers; provide accurate information on available services and encourage pursuit of programs. Complete agency intakes, establish rehab plans, secure appropriate funding, and handle insurance and medical network information. Provide manpower and assist with specialized events such as residential programs, open house, adaptive sports and recreation programs, and other rehab-related service programs. Assist and support outreach efforts—including health fairs, community presentations, agency tours—and conduct community presentations to represent Cabvi at meetings. Consult, refer, and network with primary care givers, NYSCB personnel, associated professionals, and Cabvi staff. Complete all required documentation in a timely manner, develop a weekly schedule, and document mileage and service on TICR daily. Adapt to change and complete other duties as assigned. Consistently achieve annual performance objectives. Qualifications

Minimum associate’s degree and COTA certification. Minimum of three years’ working experience in a COTA capacity. Case management or other social work experience a plus. Experience with adaptive sports and recreation, and serving individuals with vision loss and children’s recreational programming a plus. Work Environment

Extensive travel required; services may occur in consumer homes, schools, Cabvi, or the community. EEO Statement
