Per Diem Nurse Practitioner (NP)
Palliative/Hospice Care Experience Highly Preferred.
Must be able to cover San Gabriel Valley/IE Areas.
$150/visit + Administrative Compensation
Position Summary The Nurse Practitioner (NP) is an experienced licensed professional who works in collaboration with the Hospice Medical Director/Physician to deliver hospice and/or palliative care to patients and families. The NP also cooperates with the interdisciplinary Hospice team, acts as a consultant to the team, and participates in IDG meetings. The NP regularly reviews and updates the plan of care, performs prescribed medical treatments, conducts assessments and evaluations, and provides education and supportive care to hospice teams, patients, and caregivers.
Responsibilities
Conduct patient examinations and diagnoses.
Prescribe medications for pain management and symptom control.
Perform medical treatments and assessments.
Update and review care plans with the interdisciplinary team.
Qualifications
Master's or doctoral degree in nursing.
Current NP license within the state; certification in Hospice and Palliative Nursing desired and encouraged.
Minimum of two years experience as an NP within the last three years.
Seniority level: Mid‑Senior level
