Founded by a Latina grandmother/community advocate over 50 years ago in Barrio Logan, FHCSD has grown into one of the largest community health systems in the country, with over 90 sites and 1.1 million visits last year.
Job Roles
Complete all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards; documentation complies with patients' insurance requirements.
Perform initial and on‑going assessments for assigned patients.
Participate in development of patients' individualized care plans in a timely and thorough manner.
Provide treatment to assigned patients within scope of practice, meeting program/department productivity and quality standards.
Proactively utilize clinical supervision and/or peer consultation.
Demonstrate awareness of transference and counter‑transference issues and address them as they arise.
Address co‑occurring disorders, factoring them into the patient's treatment plan.
Demonstrate clinical skills and judgment necessary to implement the treatment plan.
Work effectively as a member of the multi‑disciplinary treatment team.
Communicate with providers, physicians, case managers, scheduling staff, referring providers, and outside agencies in a timely and appropriate manner.
Connect patients to internal/external resources as appropriate, and accurately document referrals.
Perform other duties as assigned.
Education/Certifications/Licenses/Registrations
Master’s Degree in Social Work or closely related field required.
Current California registration as a Registered Associate Clinical Social Worker required.
The following certifications are required:
American Heart Association healthcare provider CPR (BLS), ACLS, ATLS, or ARTT.
Possession of a First Aid/CPR certificate issued by an authorized agency.
Travel between sites occasionally required; must have a valid California driver’s license and acceptable driving record.
Experience / Specialized Skills (including Language)
Maintain good interpersonal interactions with clients and co‑workers in a multicultural setting.
Provide support, structure, empathy, sound judgment, and insight into human behavior and family relations.
Work well in both team‑based and independent environments.
Bilingual in Spanish/English or other language may be required depending on assignment.
Clinical skills appropriate to scope of practice and clinical judgment required for treatment implementation.
Demonstrated cultural sensitivity and respect for diversity.
Strong written and verbal communication skills, including with patients whose first language is not English.
Knowledge of behavior modification theories and current counseling theories and best practices.
Basic computer literacy for electronic medical record documentation and related tasks.
Work Environment FHCSD’s services are outpatient and designed for mild to moderate acuity. Therapists receive support from other therapists, case managers, a scheduling team, administrators, and a wide range of medical professionals. The Mental Health Department fosters an inclusive community that encourages teamwork and continuous learning.

Founded in 1911 as the San Diego Industrial School, it evolved into the Chicano Free Clinic and later the flagship Logan Heights Family Health Center. Over the decades, FHCSD has expanded to provide care for over 227,000 patients each year, with 91% low‑income and 29% uninsured.
Pay Range $35.00 - $39.35 per hour (base salary). Final compensation will be determined by geographic location, jurisdictional requirements, skills, education, and experience.
