Job Description The

Oncology Nurse Navigator

is responsible for navigating and optimizing the experience of prostate cancer patients as they move through the continuum of cancer care, from screening and detection to diagnosis, treatment, and survivorship. The Oncology Nurse Navigator will assist the physician and staff while working to deliver superior patient experience.

Primary Duties & Responsibilities

Act as the primary point of contact for prostate cancer patients, their families, the Urologist, and internal and external care providers.

Assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs and communicate any concerns to the Urologist.

Assist the patient and office staff in scheduling tests, procedures, appointments, and treatments.

Help develop an appropriate treatment plan in consultation with the Urologist that is understandable and feasible for the patient.

Educate and reinforce education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on a multidisciplinary care plan.

Assist patients in overcoming barriers to care (transportation, financial counseling).

Refer patients to supportive care services such as dietitians, social workers, and financial counselors.

After consultation with the Urologist, help eligible patients access appropriate clinical trials.

After consultation with the Urologist, educate patients about appropriate new technologies.

Direct patients and families to available community resources and supportive services.

Coordinate discharge planning with the Urologist and office staff.

Participate in multidisciplinary consultations/conferences and document recommendations.

Participate in prostate cancer disease planning and help ensure compliance.

Assist in the development of patient education materials, clinical pathways, and quality studies.

Work with the Urologist, Radiation Oncologist, and office staff to coordinate care and ensure timely appointments, results reporting, and communication of patient care and follow-up, which includes internal and external care providers.

Promote community education and increase prevention awareness and early detection screening programs.

Monitor and report on outcomes measures to Advanced Prostate Cancer Physician Champion, Prostate Cancer Patient Navigator, if applicable, and Program Administrator, if applicable.

Qualifications

Bachelor’s degree in a healthcare-related field preferred, except in MD.

MD Only: Registered Nurse (RN) License: Must hold a current and active RN license in MD.

Experience working in a medical or healthcare setting preferred.

Extensive knowledge of healthcare.

Urology experience preferred but not essential.

Knowledge, Competencies & Skills

Travel

Must have the ability & willingness to travel to other sites as necessary within a geographic region.

Physical Requirements For The Job

Regularly required to sit and stand for extended periods.

Involves standing, sitting, walking, bending, stooping, and twisting.

Requires full range of body motion, including manual and finger dexterity and eye-hand coordination.

May require assistance in transferring a patient.

Occasionally lifts and carries items weighing up to 40 lbs.
