Job Summary Ennoble Care is hiring for a full‑time Hospice Registered Nurse Case Manager who will work within the Burlington / Camden County, NJ region. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician, and the interdisciplinary group to provide skilled nursing assessments, planning, and care in order to maximize the comfort and health of patients and their loved ones or caregivers. The RN Case Manager will be responsible for identifying patient and caregiver needs and providing supportive care in accordance with the attending physician’s orders, plan of care, and policies and procedures.

Responsibilities

Assigned a patient caseload by the Patient Care Coordinator or designee

Primary responsibility for a patient caseload that includes assessment, planning, implementation, and evaluation phases of the nursing process

Initiate communication with attending physicians, hospice staff, and other agencies to coordinate optimal care and use of resources for patients and loved ones or caregivers

Maintain regular communication with the attending physician concerning patient and caregiver issues

Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support

Provide accurate and timely documentation in the electronic medical records

Participate in and contribute appropriate information for interdisciplinary group meetings

Coordinate physical care of the patient by teaching primary caregivers, volunteers, and employed caregivers or by providing direct care as appropriate

Meet regularly with the hospice nursing staff to review unique issues from caseloads; share professional support; and exchange feedback aimed toward enhancing professional growth

Attend patient care conferences and facilitate discussion of issues from caseload for full staff discussion, consultation, and evaluation

Supervise and document the NAC every two weeks or as assigned

Inform the Patient Care Coordinator of unusual or potentially problematic patient or family issues

Act as associate nurse when responding to patients, loved ones, or caregivers of other primary nurses during on‑call time or upon request to fill patient/family needs during the absence of other primary nurses

Provide appropriate support at the time of death and during the period of bereavement

Participate in hospice orientation and in‑service training programs for professional staff

Participate in agency and community programs to promote professional growth and understanding of hospice care

Maintain familiarity with policies of the agency and rules and regulations of state and federal bodies governing hospice care

Accept other assignments as appropriate

Qualifications

Graduate of an accredited school of nursing

Licensed in New Jersey as a registered nurse by the New Jersey State Board of Nursing in accordance with NJSA 45:11‑23 and NJAC 13:37

One year of RN experience; hospice experience preferred

Excellent assessment and problem‑solving skills

Skillful in organization and time management; knowledge of case‑management processes

Licensed driver with an insured automobile in good working order

Ability to communicate well, both verbally and in writing

Fluent in both English and Spanish
