Per diem Nursing Care Coordinator – Day Shift
Gurwin Jewish Nursing & Rehabilitation Center – Commack, NY
Pay: $63.87 – $66.82 per hour.
About Gurwin Jewish Nursing & Rehabilitation Center Gurwin Jewish Nursing & Rehabilitation Center is a 460‑bed facility located on Long Island, NY that offers a full continuum of services, including short‑term rehabilitation, long‑term skilled nursing, home care programs, an adult day health program, ventilator‑dependent care, palliative and hospice care, a Memory Care Unit, and on‑site dialysis. The Gurwin Family of Services also includes the 201‑apartment Gurwin Jewish Fay J. Lindner assisted‑living community and the Fountaingate Gardens independent‑living community on the Commack campus.
Schedule & Orientation • 7:00 am – 3:15 pm. Minimum of one weekend per month (Saturday and Sunday) and additional time as needed. • Must attend a minimum of one week of orientation (8:00 am – 4:15 pm).
Key Responsibilities
Coordinate direct nursing services to patients in collaboration with unit staff and nursing personnel.
Serve in both a managerial and clinical role, ensuring adherence to and support of the institution’s nursing practice philosophy and policy.
Collaborate with other disciplines and nursing colleagues on other shifts to promote continuity of care.
Assume administrative duties, maintain nursing staffing levels, and manage patient care requirements throughout the facility.
Required Qualifications
Graduate of an accredited School of Nursing.
Licensed Registered Nurse in New York.
Progressive experience in Nursing Administration.
Strong leadership and administrative ability.
Bachelor’s degree in nursing preferred.
