What You’ll Do
Deliver personalized, high-quality care to older adults in homes and senior living settings.
Conduct comprehensive assessments and develop individualized care plans.
Manage chronic conditions with skill and empathy.
Collaborate across clinical disciplines (primary, psych, palliative, neuro) for whole-person care.
Build trusted relationships with residents, caregivers, and community partners.
Maintain thorough documentation and consistent communication.
What You’ll Bring
Active, unencumbered NP or PA license in the state you will be practicing
Minimum of one year of clinical experience (geriatrics or primary care preferred)
Compassionate, patient-centered approach and strong clinical judgment
Valid driver’s license and reliable transportation
