Inova Health . Inova Fairfax Hospital is looking for a dedicated Experienced Registered Nurse Case Manager 1 to join the Case Management Team supporting Observation. This role will be Full-Time, Day shift: Monday - Friday 11:30am - 7:00pm with every 5th weekend and 2 holidays per year. Collect performance and outcome indicator data and assist in reporting resource and financial indicators, including LOS, cost per case, readmission rates, and more. Analyze variances from plans of care, use concurrent data to drive practice changes and positively impact outcomes. Provide discharge planning and continuity of care for assigned patients in acute and post-acute settings, initiating referrals to clinics, home healthcare, hospice, SNF, and other services. Collaborate with interdisciplinary teams, patients and families to coordinate care and ensure plans meet physical, social and emotional needs. Advocate for patients, resolve barriers to care and apply utilization management techniques to determine medical necessity. Discuss payer criteria and follow-up to resolve problems; monitor appropriateness of admissions and stays. Identify at-risk populations using screening tools, monitor LOS and resource use, and refer cases appropriately. Participate in assessment of clinical and psychosocial needs and communicate routinely with patients, families and teams. Minimum Qualifications
Education: BSN from an accredited school of nursing (ADN with completion of BSN within 5 years). Experience: Minimum 1 year of case management and/or clinical care experience. Certification: RN license in Virginia (or eNLC privilege), Basic Life Support (BLS) for Healthcare Providers upon start. Preferred Qualifications
One year of inpatient case management and discharge planning experience highly preferred; prior ED or Observation experience a plus. About Us
