Clinical Manager - Capital Caring Hospice - Washington DC - Float
Clinical Manager is responsible for the day-to-day triage of the team activities of accepting referrals, assuring timely admissions, assigning patients to appropriate team members and monitoring patient care as ordered on the interdisciplinary plan of care. The Clinical Manager conducts the IDT meetings and supervises and supports clinical staff through evaluation and field supervisory visits and assures ongoing development of the teams' clinical skills. The Clinical Manager will assure quality care and compliance with state and federal regulations, CHAP accreditation standards, and the organization’s policies and procedures.
Supervisor responsibilities: supervising Case Managers, RNs, LPNs, CNA/HHAs, Therapists, Social Workers, Grief and Bereavement Coordinators, and Chaplains.
Qualifications: - Minimum one year of Hospice/Home Health experience. - Minimum two years of management experience and at least four years of medical survival and/or oncology preferred. - RN from an NLN accredited nursing program. BSN preferred. - Licensure from D.C., Maryland, or Virginia. - CPR certification required.
Essential Duties and Responsibilities
Receives/triages inquiries from the Referral Center, staff, patients, caregivers, referral sources and community. Performs telephone assessment to determine needs of the caller. Develops and implements a plan to address the needs.
Assures that interdisciplinary team members have demonstrated skills competencies to care for assigned patients.
Assists the Executive Director as liaison between Hospice and other professionals.
Enhances the integrity and clinical practice of the team by providing role model, mentor, instructor, and resource person.
Assumes responsibility for own professional growth and development.
Evaluates own needs for support.
Develops new skills by ongoing education and maintaining knowledge of current nursing practice.
Participates in quality improvement activities, QUAPI program, hospice inservice training, serves on committees or teams as approved.
Organizational/Regulatory Compliance
Ensures compliance with federal, state and local laws, CHAP accreditation standards, Conditions of Participation, and organization policies.
Communicates with appropriate physicians.
Performs introductory (90 day) and annual performance reviews of assigned team members.
Assures licensed practical nurses, nursing assistants and other care assistants work under direct supervision of RN.
Follows all facility rules and regulations.
People and Communications
Assigns and coordinates daily schedules of clinical staff, including PRN staff.
Coordinates interdisciplinary team services, including on-call personnel.
Assists the Executive Director to assess need for new staff and recruit, interview, and hire.
Coordinates with Clinical Education staff to assure staff attend education programs.
Coordinates with staff at contract bed facilities regarding hospice plan of care.
Collaborates and communicates with facility staff and CNA regarding patient status.
Compensation and Payment $95,204.19 - $142,805.69
Additional Information This position requires consent to drug and/or alcohol testing after a conditional offer of employment and ongoing compliance with the Drug‑Free Workplace Policy.
