In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Durango, CO This role provides per‑day opportunities to deliver in‑home health evaluations for patients across Durango and surrounding areas, offering a flexible schedule that accommodates your lifestyle.

Pay & Hours Base pay range: $41.00/hr – $90.00/hr (typical range $41.85 – $90.13/hr). Anticipated weekly hours: 12. Time type: Part time.

Responsibilities

Conduct in‑home visits, performing comprehensive physical evaluations and medication reviews.

Complete diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening as ordered by the patient’s plan.

Use an iPad and intuitive clinical workflow for each evaluation.

Provide patient education and answer health questions during each visit.

Schedule and conduct virtual visits as needed.

Travel locally and within licensed states, as required.

Qualifications

Active, unrestricted license(s) in covering area(s) (multi‑state licenses preferred).

Board certification (required).

Scenarios require NP or PA; applicants must meet specialty licensure requirements.

Ability to practice within the Durango area.

Work Environment Work is performed in patients’ homes. Clinicians must be comfortable working in diverse settings, including households with pets, secondhand smoke, or potential airborne allergens.
