Physicians

serve our community by providing comprehensive direct medical care and services and working with other care team members to provide quality patient-centered care to our patients. Practiced specialists in our health centers included OB/GYN, Family Medicine, & Internal Medicine. Salary determined based on qualifications and experience.

Works with staff, as a team, to provide high-quality care to our patients by identifying and assessing health concerns, examining the patient based on the health concern, and creating a plan specific to the patient needs.

Develops and maintains an awareness of the Chronic Care Model Processes in order to reduce health disparities within our communities.

Provides clinical consultation to other staff members and supervision of staff and students as assigned.

Demonstrates adherence to National Patient Safety Goals and plan.

Participates in quality improvement activities to assess, monitor and improve program services as well as mandatory in-services and drills, attends staff meetings and other training, including appropriate age-specific educational training offered on a regular basis for professional development.

Scheduled Hours

40 Hours per Week

Monday - Friday with a weekly evening rotation and a monthly Saturday rotation

Requirements

Graduation from an accredited school of medicine and the completion of one year of residency (internship) in an approved program.

A medical license in the State of Illinois.

A completed residency in an approved program in a relevant specialty, with Board eligibility, is

preferred.

Post-residency experience in Primary Care is

preferred.

Must be board certified within 2 years of hire.
