Physical Therapist ($20,000 Sign On Bonus)

The Physical Therapist is responsible for evaluating/assessing needs of patients and formulating treatment plans. S/he provides therapy services defined in treatment plans and works cooperatively with physicians. The Physical Therapist may supervise physical therapy assistant(s).
ESSENTIAL JOB FUNCTIONS
Consults with patients to learn about their physical condition and symptoms.
Provides manual therapy as part of treatment plan when indicated.
Assesses and evaluates therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
Instructs patients and families/caregivers in the use and care of therapeutic appliances.
Determines priority needs for physical therapy.
Reports to physician patient’s reaction to treatment or changes in condition and document in patient record (EHR).
Initiates physical therapy program and coordinates with family members on home programs, exercise, and case conference.
Identifies patient and family/caregiver needs for other home health services and refers as necessary.
Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.
Performs other relevant job duties as required by manager/supervisor.
Complies with federal and state regulations.
Complies with APTA code of ethics.
Complies with continuation education requirements.
QUALIFICATIONS
Bachelor’s, Master’s, or Doctoral degree approved by the California Board of Physical Therapy.
Licensed to practice as physical therapist in California.
Must meet the educational requirements set by the California Board of Physical Therapy.
1 year of experience in outpatient setting.
Current documentation of Basic Life Support is required.
LANGUAGE
Must be able to fluently speak, read and write English.
Fluent Chinese speaker (Cantonese and/or Mandarin) preferred.
Fluent in other languages are an asset.
