Base Pay Range $110,000 – $125,000 per year.
Responsibilities In partnership with the Case Manager, you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive, and fluency disorders that affect communication. You will serve as an educator, patient advocate, and member of the interdisciplinary care team.
Qualifications
Current unrestricted CA Speech Therapist License
Current BLS/CPR Certification
Valid CA Driver's License
Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
Knowledge of state and federal home‑care regulatory guidelines preferred
Experience in home‑health care highly preferred
Experience with high acuity patients is a plus
Experience working with an interdisciplinary team highly desired
Case management fundamentals highly desired

Organized
Complete a computer literacy and competency assessment prior to scheduling an interview; this is a data point in the hiring decision process.
