Home Health Physical Therapist (PT) - Level 1
to serve patients throughout San Francisco. In this essential role, you will collaborate closely with our interdisciplinary team, including nurses, therapists, and administrative staff, to provide individualized care and ensure high-quality patient outcomes. Provide physical therapy services within the scope of practice, following individualized plans of care Deliver treatment, evaluate patient response, and report changes to the Physician, Case Manager, and/or Director of Nursing Maintain appropriate clinical records and submit documentation on company devices per organizational policy Coverage Area:
Fixed Territory .You will be assigned a specific, compact coverage area (e.g., San Francisco proper) to minimize drive time. Minimal PTA supervision Follow all company policies and procedures Perform other duties as assigned Requirements
0–1 years of clinical experience; Home Health experience preferred but not required. Ability to travel within San Francisco and surrounding areas Caring, compassionate personality with strong communication skills Schedule:
Monday – Friday only. No weekend or evening requirements. Annual Full-Time Expectation:
$135,000 – $150,000 per year Pay:
$65.00 – $72.00 per hour This is NOT a contracted position
