Primary Care Physicians: Health Advocates Delivering Patient-Centered Care
La Clínica de La Raza is a community‑based health center committed to providing culturally appropriate, high‑quality, and accessible health care to the diverse communities of the San Francisco Bay Area. For over 40 years we have advocated for and created a health home for many who have been denied access to care. We understand that healing our communities requires addressing the economic and social factors that affect health, and we provide services in multiple settings to link patients to our health center for preventive and primary care. With 90,000+ individuals served, we coordinate and connect patients to a broad network of services to improve and maintain their health and wellbeing across 35 sites in Alameda, Contra Costa, and Solano counties.
