Sr. Nurse Practitioner, Community Access & Mental Health Program Manager (DOM-Infectious Diseases)
The Spot mobile clinic is operated through a partnership between Johns Hopkins University School of Medicine and the Baltimore City Health Department. The Spot provides integrated care for people who use drugs across Baltimore City, including buprenorphine management, infectious diseases testing and treatment, wound care, harm reduction services, and case management. This new position will help lead efforts to ensure Spot patients are connected to health and social services, that their social determinants of health are addressed, and that they are provided with mental health services on‑site or in partnership with outside providers. The role will supervise a multidisciplinary team of case managers, patient advocates, and part‑time mental health providers.
Specific Job Duties And Responsibilities
Provide behavioral health services to Spot clients.
Directly supervise case managers, patient advocates, and part‑time mental health providers, including LCSWs, psychiatric nurse practitioners, and other staff.
Develop and modify protocols for case management services, including scope of services, initial patient assessment, follow‑up planning, and documentation in the electronic medical record. Develop protocols for patient advocacy services, including assistance with health care navigation, accompaniment, and supportive services.
Partner with Johns Hopkins Department of Psychiatry and other outside providers to establish on‑site and/or telemedicine mental health services for Spot patients. Develop service delivery protocols and supervision structure.
Monitor programmatic activities and develop a continuous quality improvement plan for case management, patient advocacy, and mental health services.
Coordinate and deliver training sessions for direct‑report staff, and for other Spot team members and partner organizations relating to mental health, case management, or other relevant topics.
Provide onsite support for registration, linkage to care, and referrals.
Offer technical support to the front‑of‑house team for registration and insurance processes.
Support leadership with administrative duties related to grant and budget management, including developing reports for periodic internal review and grant reports.
Participate in data analysis and manuscript preparation alongside the executive team. Support the development, implementation, and management of any approved, service‑related research.
Collaborate with a multidisciplinary team at the BCHD to develop and implement city‑wide efforts related to opioid overdose prevention, HIV, viral hepatitis, and other relevant topics.
Collaborate with other BCHD teams, including Community Risk Reduction, Overdose Prevention Team, Sexual Health Clinics, Tuberculosis, and HIV/STI Prevention as needed.
Work with BCHD on implementation and optimization of electronic medical records (OCHIN/Epic) and billing processes.
Moderate patient complaints of staff or services in a mindful and empathetic manner.
Work with learners from various clinical institutions in Baltimore for clinical and research rotations.
Maintain flexibility to expand the scope of work.
Other duties as assigned.
Clinical Duties And Responsibilities
Provide direct care to Spot patients within their scope of practice.
Be available for coverage of case management duties as needed.
Participate in research activities authorized by JHU and BCHD.
Provide emergency services to BCHD or JHU as needed.
Maintain a current and valid license in accordance with the respective governing board.
Minimum Qualifications
Master’s Degree in Nursing; Registered Nurse license and Certified as a Nurse Practitioner. Must be licensed as an NP in the State of Maryland.
Three years of experience in a related clinical area and obtained proficiency in the area of specialty.
Residency or fellowship programs in the area of specialization may substitute for some experience.
Preferred Qualifications
Psychiatric nurse practitioner with years of experience in case management.
Experience in supervising multidisciplinary staff.
Knowledge of spoken Spanish.
