Position Title Center Medical Director

About The Role The Center Medical Director leads the clinical team at Oak Street Health to provide exceptional patient care and create a great workplace. The MD will practice primary care in the clinics at 80% clinical time and 20% administrative time. The role includes leadership responsibilities such as setting the standard for thoughtful primary care, mentoring providers, and engaging in organization‑wide strategic initiatives.

Core Responsibilities

Support organizational strategy by collaborating with Practice Manager and Outreach Manager/Director to achieve objectives such as Best Care Anywhere, Unmatched Patient Experience, Exceptional Growth, Great Place to Work, and Leadership in DE&I.

Drive the implementation of OSH strategic initiatives and follow direction from regional leadership.

Communicate organizational information to care team members and elevate team concerns to the organization.

Coordinate and lead onboarding of new providers.

Model exemplary medical care, support the care model implementation, and ensure evidence‑based practices keep patients healthy.

Implement Oak Street’s clinical guidelines at provider and center levels.

Support high‑quality primary and secondary preventative care and the full‑risk value‑based model.

Assess and address root causes when metric goals are not met.

Create and maintain a culture of coaching and growth for providers and care teams.

Conduct performance reviews, discuss goals, and develop plans for achievement.

Celebrate successes, communicate improvement opportunities, and maintain the Live‑at‑Oak culture.

Perform additional duties as assigned.

Ideal Candidates

M.D., D.O., A.P.N, or P.A. graduates.

Board certification in Internal Medicine or Family Medicine (for M.D./D.O.) or in Family, Adult, or Gerontology (for A.P.N.).

Experience in clinical leadership, coaching providers, and metrics‑driven care.

Passionate, motivated, and eager to impact a fast‑growing organization.

Commitment to older adults and low‑income patients.

Comfortable with ambiguity and adept at problem solving.

Calm under pressure, guiding others to effective solutions.

5+ years of outpatient practice experience preferred.

Additional education (MBA, MPH, etc.) and executive roles welcomed.

U.S. work authorization.

Pay Range:

$174,070.00 – $374,920.00 (base salary depends on experience, education, geography)
