DUTIES & RESPONSIBILITIES
- Job Title: Family Medicine or Internal Medicine Physician
- Job Type: Permanent Full-Time
- Location: Hobart, IN
- Service Setting: Outpatient
- Schedule: Monday Friday, standard clinic hours
- On-Call Expectations: 1:10 rotation
- Start Date: Upon completion of credentialing (90120 days) PATIENT POPULATION & SCOPE
- Patient Demographics: Adults (Family Medicine may also see Pediatrics and Geriatrics)
- Average Daily Volume / Census: 2026 patients per day
- Consults & Follow-Ups: Yes
- Case Mix/Clinical Focus Areas: Preventive care, chronic disease management, general internal medicine, and family practice FACILITY & TEAM DETAILS
- EMR System: Epic fully integrated HIMSS Stage VII
- Specialty Backup: Collegial system physicians and subspecialty support
- Support Staff: Multidisciplinary team with APPs, nurses, and support staff
- Leadership & Reporting Structure: Reports to practice Medical Director

- DEA & Certifications: DEA, CSR, BLS required
