Requirements:
  • HI License or IMLC
  • BC
  • ACLS
  • CSR/CDS
  • DEA
  • Health Immunization/vaccination (with dates)
  • Recent NPDB
  • CV in MM/YYYY format
  • Gaps of 30+ days must have explanation provided
