Requirements:
- Active CO license or IMLC (LOQ)
- BC in Otolaryngology (Head & Neck Surg)
- BLS
- DEA (will consider a transferred DEA, however, must keep this DEA for the duration of the assignment)
- Available for any of the dates
- Understands & completes the EMR training requirements
- Clean record
- Local/drive-in (highly preferred)
