The Clinical Pharmacist is responsible for planning and providing clinical pharmacy services, disease management, Medication Therapy Management, and other patient/staff education programs. The pharmacist ensures compliance with current regulations and national best practices, documents outcomes, supervises students, and participates in health center committees and initiatives. The pharmacist assists the Pharmacy Manager in daily pharmacy operations, including dispensing responsibilities.
Key Responsibilities
Recognizes, communicates, and collaborates to resolve drug‑drug and drug‑food incompatibilities.
Designs, implements, and supervises evidence‑based drug monitoring plans based on provider diagnoses.
Identifies, communicates, and assists in resolving medication therapy problems.
Performs appropriate testing in accordance with CLIA and facility standards.
Provides education on medication in the community.
Participates in outcome and drug research projects.
Assists the Director of Pharmacy with planning and operation of the Pharmacy Department.
Assists in training, supervision, and evaluation of technicians and pharmacy support staff.
Performs all other duties as required.
Per Diem Requirements The position requires flexible availability Monday through Friday 8:30 a.m.–5 p.m. and Saturday 8:30 a.m.–12:30 p.m., with shifts of up to 8 hours per day during the week, as needed. Coverage is required for colleagues’ sick days and vacation time. To maintain Per Diem status, the Clinical Pharmacist must work a minimum of 4 shifts per month, not to exceed 19 hours per week.
Requirements / Qualifications
Degree from an accredited College of Pharmacy required; residency preferred.
Licensed or eligible to practice pharmacy in the state of Connecticut.
Participation in the American Society of Health System Pharmacists or another pertinent national pharmacy organization preferred.
Experience and familiarity with Joint Commission standards preferred.
Ability to communicate in a professional and effective manner with staff and patients concerning drug therapy is required.
