Metro is looking for a motivated and passionate FULL TIME Physical Therapist to provide in-home care. All levels of experience are encouraged to apply and the right new graduate will be trained. Come be a part of a growing organization that is progressive, supportive, and innovative. What We Offer

Responsibilities
Accept and treat patients within your designated geographic area Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans Develop a plan of care and treatment goals in consultation with the physician, patient, and family Plan, prepare and implement customized therapy programs to restore function and promote independence Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques Provide timely and accurate documentation for each visit Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning Deliver care in compliance with agency policies and professional practice standards Qualifications
Physical Therapist Program Graduate Licensed Physical Therapist in the state of New York or eligible Mandatory certifications: First Aid, AED, & CPR Home health experience preferred but not required – mentorship provided for documentation and compliance Ability to travel to patients’ homes within the assigned geographic territory Compensation
The anticipated base salary range is $85,000.00–$100,000.00. Salary is based on relevant experience, knowledge, skills, other job‑related qualifications, and geography. The position is eligible for incentive compensation subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company’s plan.
