Position Summary / Duties
Provide full social work coverage including psychosocial assessments, determining need for financial/medical assistance, supportive counseling, and arranging concrete services.
Participate in counseling and determining suitability for treatment modalities.
Coordinate interdisciplinary consultation.
Evaluate patients within assigned social work discipline.
Perform ongoing assessment of social, psychological, environmental, and financial factors for ESRD patients.
Develop social work treatment plans and assist in multidisciplinary program evaluation, development, and care planning.
Counsel patients, families, significant others, and staff on adjustments to chronic illness and lifestyle changes.
Obtain, review, and distribute educational materials as needed.
Coordinate referrals and follow-up with community agencies.
Ensure patient access to appropriate resources and maintain referral relationships.
Collaborate with other services for permanent transfers and transient care.
Monitor and supervise transportation referrals for timely service.
Communicate regularly with financial offices to support Accounts Receivables.
Assist staff in reporting abuse, neglect, or domestic violence.
Serve as a resource to staff, patients, families, and caregivers.
Perform other duties as requested.
Required Skills & Experience
Master’s degree: No experience required.
Internship or experience in hospital/medical setting preferred.
Required Education
Master’s Degree from a Council on Social Work Education–accredited program.
Completion of coursework with specialization in clinical practice.
Required Certifications & Licensure
New York State Social Worker License.
