Position Summary for Admissions Registered Nurse (RN):

An admissions nurse is an RN and an interdisciplinary team member whose primary responsibility is to present ANX Hospice Care services to patients and families in a manner that results in the admission of appropriate patients. The admissions nurse performs consultation and admission visits to assess the patient's needs and completes admission assessment and contractual agreement documentation for hospice.

Responsibilities:

Possess an understanding of the hospice program and benefits.

Knowledge of intake, admission, and transition to care processes and a commitment to developing skills in these areas.

Evaluate patients for hospice appropriateness, including medical records review as requested.

Document assessment, current medications, and initiate appropriate care plans.

Write and initiate a plan of care that establishes goals based on nursing diagnoses and incorporates the patient and family in the planning process.

Assess patient’s clinical status, plan, teach, and implement immediate interventions on the day of admission.

Coordinate medications and durable medical equipment needs for same‑day delivery.

Coordinate with attending physician and hospice medical director regarding patient admission and follow‑up needs.

Communicate admissions information, issues, and concerns in inpatient care meetings to ensure smooth transition and continuity of care, including a detailed report to the assigned case manager and social worker.

Perform routine and unscheduled nursing visits when required and serve as a backup to other field staff when there are no admissions.

Use interpersonal skills to help educate and address questions the patient and/or family has concerning hospice care.

Participate in the weekend on‑call rotation, holidays, and after‑hours as needed.

Provide admissions at the patient’s home, hospital, board, or skilled nursing facility.

Be available for comprehensive nursing evaluations, demonstrate flexibility in scheduling, and readily accept new admissions and emergency assignments, participate in QAPI, and adhere to high standards of professional conduct.

Minimum Qualifications:

Able to work on weekends in both San Mateo and San Francisco counties.

Current CA Registered Nurse (RN) license.

Current CPR card for Healthcare Providers.

At least 1 year of registered nurse experience in hospice care.

Licensed driver with a reliable, insured vehicle in good working order.

Understanding of hospice philosophy and experience with terminally ill patients.

Demonstrated nursing skills, coping with family emotional stress, and tolerance of diverse lifestyles.

Excellent observation, problem‑solving, verbal and written communication skills; nursing skills per competency checklist.

Self‑directed and able to work with minimal supervision.
