Job Summary Reporting to the Director of Patient Safety, the Patient Safety Specialist promotes a safety-supportive culture, applies practice-based medicine, and implements the UCSF Patient Safety Plan. The role identifies and anticipates patient safety opportunities, determines risk exposure, and coordinates investigations, analysis, and responses to quality/patient safety issues and significant care trends. The specialist conducts surveillance, analysis, and data reporting while meeting regulatory and accreditation requirements, serves as a subject matter expert, collaborates with leadership and operational partners, and supports educational programs and performance improvement initiatives.
Responsibilities
Coordinate efforts to improve patient safety organization-wide by identifying problem areas, assessing and escalating to leadership, and overseeing projects related to regulatory compliance.
Participate in and facilitate Root Cause Analyses, FMEA, Culture of Patient Safety Surveys, and other event review activities.
Maintain collaborative relationships with nursing and physician leadership to develop, implement, and monitor corrective action plans for patient safety events.
Monitor patient safety data to interpret trends and assess the overall health of the program.
Serve as a lead or resource on patient safety operations, projects, and initiatives, and manage complex or high-risk projects.
Provide clinical licensure expertise on events and be onsite at least three days per week.
Required Qualifications
Minimum four years of related experience.
Bachelor’s degree in nursing or health discipline, or licensed clinician in California (e.g., Nursing, Pharmacy, Respiratory, Physical Therapy) or equivalent combination of experience/training.
Clinical license (RN, PharmD, PA, MD, RT, PT, OT, etc.).
Strong skills in data analysis, project management, facilitating group work, and hospital systems improvement.
Thorough knowledge of clinical improvement issues, patient safety methodologies, regulatory requirements, and policies.
Strong project management and time‑management skills with the ability to handle competing priorities and complex issues.
Strong interpersonal skills for collaborating with clinical, technical staff, external regulatory agencies, and staff mentoring.
Proven ability to convey complex clinical, technical, and educational information verbally and in writing, producing high‑quality reports and presentations.
Knowledge of clinical and business software packages and specialized applications used in patient safety and quality improvement systems.
Experience with adverse event monitoring and remediation, including root cause analysis and failure mode effect analysis.
Preferred Qualifications
Experience with electronic health records, including CPOE, decision support, and clinical documentation.
Experience in a large multi‑facility health system or academic institution.
Experience in perioperative services or operating room settings.
Master’s degree or equivalent in a healthcare‑related field (nursing, public health, health administration, or related).
Certified Professional in Patient Safety (CPPS) or Certified Professional in Healthcare Quality (CPHQ).
Lean/Six Sigma certification.
License/Certification
Clinical license (RN, PharmD, PA, MD, RT, PT, OT, etc.).
