Home Health RN Case Manager | Details and Compensation:

Location: Baltimore, MD – 100% Field Work (covering Baltimore City, Baltimore County, with possible travel to AA and Howard Counties) Payrate: $56.15/hr Required Availability: Monday–Friday, 8:00 AM – 5:00 PM | Possible On-Call

Home Health RN Case Manager | Summary and Highlights:

The

Home Health RN Case Manager

will conduct comprehensive assessments of high-risk members in their homes and develop individualized care plans.

This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Home Health RN Case Manager | Responsibilities:

Conduct comprehensive assessments of high-risk members in their homes. Develop individualized care plans and collaborate with primary care providers. Ensure access to necessary non-medical support (transportation, housing, etc.). Coordinate transitions of care and engage with interdisciplinary teams. Prevent and respond swiftly to adverse patient outcomes. Maintain detailed clinical, functional, and fiscal records. Serve as an advocate for patients and protect their confidentiality.

Home Health RN Case Manager | Requirements :

Education: Associate’s or Bachelor’s degree in Nursing or related field Attendance: Commitment to mandatory attendance during the first 90 days. Active RN license in the state of Maryland OASIS and wound care experience – required HomeCare HomeBase knowledge – preferred but not required CPR/BLS certification Valid driver’s license, insurance, and a reliable vehicle Case Management Certification (preferred) Strong clinical judgment and ability to analyze complex medical data
