Role Summary: The Community Outreach Specialist II will work with the MHP Care Coordination team to ensure that patients receive the resources and services they need. This position will work with practices to coordinate care and meet network performance goals. The role is also responsible for assisting patients with needed support services to achieve personal goals and optimal health. Additionally, this position will act as a liaison between MHP patients, healthcare providers, and various groups and services in the community. What qualifications you will need: Required Educations: Bachelor degree or 3 years relevant work experience. Preferred License: Community Health Worker, current basic cardiac life support (BLS), Certified Health Education Specialist (CHES) or Peer Support Specialist Required Experience: Three years of professional work experience of which at least one year must be in health/social services related. Preferred Experience: Working with high-risk, vulnerable, or uninsured population, Spanish-speaking. Understanding and sensitivity regarding challenges facing low-income communities, Entry-level proficiency in another language such as Spanish or Russian preferred. Computer proficiency - electronic health record experience preferred.
