About the Role:

The Intensive Services Case Manager will provide case management services to children and families who suffer from emotional, behavioral and mental health challenges, in order to facilitate safety, stability, and well-being. Provided services consist of therapeutic interventions that facilitate illness self-management, skill building, identification and the use of natural supports, and the use of community resources. Services will be provided at the agency, and in the home and community when necessary. The Intensive Services Case Manager will also be working in conjunction with the Intensive Services Coordinator to interface with the State of Vermont, DMH, DCF, hospitals, crisis teams, and community partners in order to provide and coordinate community-wide support children and families with complex needs.

Principal Responsibilities: Provide case management, outreach, and aftercare services. Provide intensive services case management for high-acuity clients as assigned by the program manager. Provide and maintain collaborative communication and relations with internal and external partners, including DMH, DCF, hospitals, crisis teams, schools, and other community partners. Provide basic living and social skills building, supportive counseling, and service coordination, consultation, and community education in office, community and home-based settings. Maintain a caseload of Community Response and Intensive Services cases. Assess clients' functioning, strengths, needs, and diagnosis. Develop treatment plans collaboratively with clients by defining goals, specific outcomes, and time frames for treatment.

Qualifications: Bachelor's degree in counseling, psychology, social work, or human services required. Experience working with children and families, providing therapeutic intervention services. Must be comfortable providing home visits in a variety of locations throughout Rutland County. General knowledge of community resources, standard models of service coordination, and/or experience working in a variety of human services settings. Competent listening, communication and writing skills. Able to handle stressful situations in a calm and professional manner. Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont. *Successful completion of a criminal history background check after hire.
