This is a daylight position with an occasional night/weekend 20 hours per week. $15.25/hr

Position Summary

The Activities Assistant is responsible for assisting with the overall planning and implementation of a varied activity program designed to meet the social, physical, intellectual, recreational, and spiritual needs of residents.

Qualifications: Experience in a long-term care activity department preferred Background in music, art,drama, and dance therapy preferred High school diploma or equivalent Caring, compassion, and understanding for the elderly Activity certification or course completion preferred

Essential Duties and Responsibilities

Plans, coordinates, communicates, and conducts individual and group activities determined by the resident, resident council, resident care plans, family suggestions, the Director of Activities and other team members.

Documents activity attendance, individual visits, level of participation and information required for validation determined by the facility and regulations.

Conducts an on-going assessment of the physical, intellectual, social and spiritual needs of the residents through observation and written assessments. Reports any changes in status to the Director of Activities and nursing department as needed.

Maintains flexible schedule; includes evenings, weekend days and holidays.

Transports residents to and from activities and events within and outside the facility.

Coordinates volunteers to support the activities program

Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures. Maintains confidentiality of resident and employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.
