Job Summary: The Skilled Home Health Scheduler is responsible for coordinating and maintaining accurate staff schedules for skilled nursing and therapy visits, ensuring timely delivery of home health services in compliance with physician orders and agency policies. This role requires strong organizational and communication skills, along with the ability to multitask in a fast-paced environment.

Key Responsibilities: Schedule and coordinate skilled nursing, physical therapy, occupational therapy, and other clinical visits according to the care plan. Match clinicians with patients based on location, skill set, availability, and patient needs. Communicate with clinicians, patients, and referral sources to confirm visits and manage scheduling changes. Monitor and maintain compliance with physician orders, frequency, and documentation timelines. Collaborate with case managers and clinical supervisors to ensure smooth care transitions and continuity of care. Input and update scheduling data in the EMR system accurately and in real time. Respond promptly to missed visits, visit reassignments, and urgent scheduling needs. Maintain communication logs, scheduling reports, and patient visit compliance metrics. Ensure all visits are staffed within CMS and agency time requirements. Qualifications:

High school diploma or equivalent required; associate or bachelors degree preferred. Minimum of 12 years of experience in home health scheduling or a related healthcare administrative role. Knowledge of home health services, including Medicare/Medicaid regulations and OASIS documentation. Proficiency in scheduling software (

Homecare Homebase ). Excellent verbal and written communication skills. Strong attention to detail, organization, and problem-solving abilities. Ability to work independently and as part of a collaborative team.

Preferred Skills:

Bilingual (English/Spanish or other language) is a plus. Familiarity with HIPAA regulations and patient confidentiality protocols. Customer service experience in a healthcare setting.
