Description Home Health Physical Therapist delivers rehabilitative therapy to patients in their homes to improve mobility, strength, and functional independence. They perform detailed assessments, develop personalized treatment plans, and provide therapeutic exercises and interventions. The PT tracks progress, adjusts therapy as needed, and ensures accurate clinical documentation. They also educate patients and caregivers on safe mobility techniques, equipment use, and long-term health maintenance.
Responsibilities
Assess, plan, organize, and participate in rehabilitative programs that improve patient mobility, relieve pain, increase strength, and correct disabling conditions resulting from disease or injury.
Gain confidence and cooperation from the patient, their family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication.
Adhere to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care.
Provide clinical supervision of assistants and aides, and serve as clinical instructor for assigned students.
