We are currently seeking a compassionate, organized, and team-oriented individual to join our Hospice team as a Full‑Time or Part‑Time (Per Diem) RN in San Mateo.
Position Overview We are seeking an experienced, self‑driven RN to join our dedicated Hospice team and provide exceptional care throughout the region. You will evaluate patients’ health, create and implement care plans, and track their progress to maintain the highest standards of care. You’ll work closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

Responsibilities
Provide nursing care within scope of practice, following the patient’s individualized care plan in coordination with the Interdisciplinary Team.
Administer treatments, monitor patient conditions, and report signs or symptoms indicating a change in patient condition to Physician, Case Manager, or Director of Nursing as needed.
Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
Supervise Licensed Vocational Nurses (LVNs) when assigned.
Comply with all company policies and procedures to ensure high standards of care.
Requirements
Current RN license to practice in California – required.
Minimum 2 years of hospice or hospice‑care experience – required.
Current California driver’s license.
Ability to travel within a 15–25 mile radius on a regular basis.
