Job Summary
The Registered Professional Nurse is responsible for prescribing, delegating, and coordinating all nursing care using standards of care and practice.
Provides comprehensive mental health nursing care to adult psychiatric and substance abuse patients aged 16 and older.
Functions within the nursing process in collaboration with the healthcare team to meet patient and family needs.
Duties & Responsibilities Assessment
Assesses physical, social, and psychological status through observation, patient interview, and review of medical data.
Documents a complete nursing assessment including fall risk, pain, suicide risk, nutrition, psychosocial and environmental factors, educational level, and discharge needs.
Identifies care needs related to substance abuse, prolonged neuroleptic use, and mental illness.
Continuously reassesses patient condition and communicates findings with the healthcare team.
Nursing Diagnosis
Formulates, prioritizes, and documents nursing diagnoses aligned with assessment data.
Includes physiological, psychosocial, emotional, and educational needs.
Planning
Creates and documents patient-centered, measurable goals aligned with nursing diagnoses.
Identifies patient education needs and discharge planning requirements on admission.
Intervention/Implementation
Collaborates with the interdisciplinary team to execute the plan of care.
Provides nursing care including identifying withdrawal symptoms, managing agitated or suicidal patients, administering medications, and managing psychiatric emergencies using non-violent crisis intervention.
Maintains infection control standards.
Provides health teaching and counseling individually and in groups.
Participates in treatment planning, medical rounds, and interdisciplinary meetings.
Performs procedures per mental health nursing standards and demonstrates competency.
Investigates, resolves, and documents unusual occurrences.
Delegates and guides ancillary personnel appropriately.
Performs other duties as required.
Evaluation
Interprets, communicates, and documents patient responses to nursing and medical interventions.
Revises the care plan based on evaluation and interdisciplinary input.
Evaluates patient and family understanding of discharge teaching.
Physical Demands
Includes standing, walking, sitting, talking, hearing, bending, and some repetitive motions.
Reasonable accommodations may be made if they do not compromise patient care.
Required Skills & Experience
Knowledge in PMCS.
Inpatient and Psych ER experience required (per schedule notes).
One year medical/surgical nursing experience preferred.
Required Education
NYS RN License.
BCLS required.
Preferred Education
Bachelor’s Degree in Nursing preferred.
Required Certifications & Licensure
NYS RN License
