Are you a clinically skilled and compassionate Nurse Practitioner with a passion for guiding patients and families through life’s most meaningful transitions? As a Hospice Transition Specialist with Bristol Hospice, you’ll play a vital role in providing high-quality palliative and hospice care for patients with chronic, complex, or life-limiting conditions. In this community-focused role, you’ll collaborate with hospitals, physicians, and care teams to ensure patients receive the comfort, dignity, and support they deserve throughout their journey.

Travel : You will be required to use your own vehicle to travel throughout the Bristol Hospice service area (mileage reimbursement applies).

An Average Day

Begin the day reviewing patient referrals and coordinating with hospital staff and interdisciplinary teams to identify individuals appropriate for hospice or palliative care.

Conduct bedside palliative care evaluations, including patient and family assessments, reviewing medical records, and identifying goals of care.

Collaborate closely with physicians, nurses, and care coordinators to develop, communicate, and implement individualized treatment and transition plans.

Facilitate patient transitions from hospital to home or inpatient hospice by coordinating transportation, DME, medications, and admission scheduling.

Provide education to patients, families, and hospital staff on hospice philosophy, services, and benefits to ensure understanding and comfort with the care transition.

Serve as a clinical and educational liaison within assigned hospitals and facilities, conducting ongoing in-services and education sessions on hospice eligibility and best practices.

Maintain accurate documentation and compliance with collaborative agreements, regulations, and organizational standards.

Participate in staff meetings, community events, and internal training to promote continuity of care and professional growth.

Monitor and respond to referral source requests, ensuring timely communication and seamless coordination of care.

Other duties as assigned.

Requirements

Graduate of an accredited School of Nursing and Nurse Practitioner Program.

Current state licensure as a Nurse Practitioner.

Minimum one (1) year NP experience and one (1) year hospice or palliative care experience.

Accreditation from an approved certifying body for advanced practice nursing as required by the state.
