Position Overview We are seeking a compassionate and dedicated Case Manager to support clinical operations for our Silver Spring office. This role is ideal for a nurse who is passionate about coordinating patient care, supporting field staff, and ensuring high-quality, patient-centered services. As Case Manager you will play a vital role in fostering communication, promoting patient well‑being, and ensuring compliance with regulatory standards and company policies.

Responsibilities

Oversee and coordinate patient care for the Silver Spring office.

Provide support, guidance, and communication to clinical staff to ensure high‑quality care delivery.

Collaborate with interdisciplinary teams to create individualized care plans that promote patient health, independence, and dignity.

Monitor and evaluate patient outcomes, implementing follow‑up or care adjustments as needed.

Ensure compliance with all regulatory and accreditation standards.

Support staff through ongoing education, training, and professional development.

Build strong relationships with patients, families, and community partners.

Qualifications

Active RN license in Maryland.

Minimum of 3 years of clinical experience in home health or a related healthcare setting.

Experience in case management or care coordination preferred.

Strong organizational, communication, and interpersonal skills.

A passion for supporting staff and fostering a collaborative team culture.
