Physician Assistant – Key West
Mount Sinai Medical Center (Key West, FL) is seeking a Physician Assistant to join our dedicated team.

Responsibilities
Under physician direction, oversee and medically manage high‑risk geriatric participants.
Conduct initial, semi‑annual, unscheduled assessments, and additional physical assessments as needed.
Assist in daily oversight of the Eldercare Clinic.
Perform rounds in contracted nursing homes, hospitals, and other care sites.
Conduct home visits for medical interventions.
Participate in development and implementation of participant plans of care with the interdisciplinary team.
Maintain regular attendance at interdisciplinary team meetings; communicate participant changes and coordinate 24‑hour care delivery.
Integrate primary care treatment plans into overall care plans.
Interact with team members to meet emergent and acute needs.
Review all hospital, ED, or other acute admissions; assist discharge planning for safe transitions.
Provide preventive health care: immunizations, screenings, monitoring.
Prudently prescribe medications, therapies, and treatments.
Demonstrate advanced knowledge of diagnostic and therapeutic procedures for at‑risk participants of the Eldercare PACE program.
Interview patients and complete comprehensive medical histories and physical examinations within PACE regulations and Eldercare policies.
Qualifications
State of Florida Advanced Practice Registered Nurse License; ACLS Certification.
Advanced Practice Registered Nurse, Geriatric Certification preferred. Master of Science in Nursing.
At least 1 year of experience as an APRN (preferable), primary care, or other medical specialty; 5 years experience as an RN and 1 year working with frail or elderly.
