JOB SUMMARY AND QUALIFICATIONS : The Targeted Case Manager’s primary duty is to provide case management services to families in the Family Recovery Court Program. TCM will assist the individual/family with obtaining access needed to support their needs.
The Case Manager must be able to assist the individual/family with obtaining needed access to housing, medical, social, educational and other identified services.
RESPONSIBILITIES: Provide case management services for families that are targeted at the identified needs of the individual and/or family. Conduct comprehensive assessments, and reassessments to determine the need for housing, medical social, education and other identified services.
Develop and individualized case plan for each individual and/or family that is based on the assessment. The plan will specify the goals and activities that address housing, medical, social, educational, and other services identified Administer Program Evaluation Assessments according to a pre-determined schedule. Administer observed Urinary Drug Screens weekly. Refer the individual/family to services that address the areas identified in the assessment. This may include scheduling appointments for the individual/family and ensuring transportation is available for appointments. Monitor and follow-up with the individual, family members and other service providers. Update the case plan as needed. Identify community resources and develop cooperative relationships with other social service providers Represent agency programs, policies and procedures accurately in the community and with other social service agencies. Maintain accurate case records, which include proper documentation of all meetings with clients. Ensure that all documentation complies with KY Medicaid and accreditation standards. Attend Family Court and Family Recovery Court dockets weekly. Conduct Life Skills classes with participants in Phase 3 of the program. Participate in weekly case conferencing and provide updates on all assigned individuals and/or families. Participate in bi-monthly supervision with TCM billing supervisor. Participate in weekly supervision with direct supervisor. Participate in monthly staff meetings and/or training courses. Attend annually required training to maintain TCM certification. Maintain certification requirements for TCM pursuant to Kentucky Board regulations. Assist with special projects assigned. Assist with reporting requirements as assigned.
