Charlottesville , VA 22911 – 100% On-site (covering Charlottesville to Staunton)

Payrate:

$56.14/hr + $0.45 mileage reimbursement Required Availability:

Full-Time | Monday–Friday, 8:30 AM–5:00 PM Flexible schedule but will need to meet 6 points per day On-call required

1 week per month

(light call volume)

Home Health Case Manager (RN)

role supports high-risk members through comprehensive assessments, interdisciplinary collaboration, and coordinated care planning. You’ll work independently in the field while partnering closely with primary care teams to ensure members receive appropriate, cost-effective, and supportive services. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
