Job Summary The
Senior Cloud Engineer (.NET/C#)
will serve as a key technical contributor on a mission-critical healthcare interoperability initiative supporting CMS Interoperability Phase 3 (CMS-0057-F). This role focuses on designing, developing, and deploying secure, scalable cloud services that enable compliant healthcare data exchange, directly impacting how patient data is shared across the healthcare ecosystem.
Key Responsibilities
Develop and own the
central Proxy FHIR Service , a C# serverless function acting as the secure gateway for all interoperability APIs.
Engineer robust
security and authorization
mechanisms, including validation of external Okta JWTs, OAuth 2.0 scope-based authorization, and secure internal token exchange.
Design and implement
intelligent routing logic
to support complex Prior Authorization workflows, differentiating between CRD, DTR, and Client requests.
Build resilient
asynchronous background jobs , including:
Synchronization of member consent choices (opt-in/opt-out) from Salesforce to a centralized repository.
A long-running payer-to-payer data orchestration process involving external API calls, FHIR bundle parsing, and data transformation for claims systems.
Provider-patient attribution processing using historical claims data.
Ensure all data processing aligns with
HL7 FHIR US Core Implementation Guide v7.0.0
standards.
Collaborate with solution architects, business analysts, and QA engineers to translate functional requirements into scalable, production-ready cloud solutions.
Required Experience
5+ years of professional software development experience using
C# and the .NET Framework .
Hands‑on experience with
cloud platforms
(Azure, AWS, or similar), particularly serverless technologies such as Azure Functions or AWS Lambda.
Strong understanding of
RESTful APIs , OAuth 2.0, JWT, and backend service security.
Proven ability to design scalable, reliable backend systems and asynchronous processing workflows.
Experience interpreting and implementing detailed technical specifications and functional requirements.
Nice‑to‑Have Experience
Healthcare technology experience, especially
HL7 FHIR
interoperability standards.
Familiarity with
Da Vinci Implementation Guides
(CRD, DTR, Client) for prior authorization.
Experience integrating with
Okta
or similar identity providers.
Salesforce API integration experience.
Knowledge of API gateway patterns and cloud‑native healthcare data platforms such as
AWS Health Lake
or
Azure Health Data Services .
