On An Average Day You Will
Receive case referrals and review available patient information related to the case, including home visits, to determine hospice/home care needs.
Assign appropriate hospice personnel to referral case as needed.
Confer with Medical Director and Attending Physician regarding any questions as to an individual’s eligibility for services.
Review and evaluate each case through a variety of means such as home visits, conferences, records review, and the services provided by clinicians.
Review patient’s medical diagnosis, prognosis, medications, procedures and clinical course.
Discuss and verify impressions, instruct and guide clinicians to promote more effective performance and delivery of quality home care services.
Be available, during operating hours, to assist clinicians as appropriate.
Conduct clinical record reviews and communicate findings and recommendations to the Clinical Director and hospice personnel, as defined by the QAPI Program.
Assist in the screening and interviewing process of new employees and make recommendations for employment of individuals.
Assist in the orientation of new employees, as needed.
Assist the Director of Patient Care Services and other supervisory hospice personnel in the planning, implementation and evaluation of in‑service and continuing education programs.
Assist in the formulating, revising, implementing and evaluating organization policies, procedures, goals and objectives – both short‑ and long‑range.
Comply with accepted professional standards and principles.
Assess the health needs of the community and assist in the development of plan(s)/program(s) to meet needs.
Participate in public relation and community activities that promote the organization’s role as an effective member of the hospice and health care delivery system.
Promote customer service orientation to all hospice personnel.
Participate actively in quality assessment performance improvement teams and activities as directed by the Director of Patient Care Services in accordance with the QAPI Program.
Perform other duties and activities as delegated by the Director of Patient Care Service.
Additional duties as assigned.
Qualifications
A Registered Nurse license in good standing to practice professional nursing in the state.
Must have previous experience in a hospice home care setting with two years of management or supervisory experience.
An enthusiasm for providing the best possible care to patients and their families.
Understanding of the hospice philosophy and issues of death and dying.
Excellent observation, verbal, and written communication skills.
A valid driver’s license.
Current CPR certification.
