Overview
Develops and organizes the program of psychosocial services for children with Hem/ Onc disorders and their families in the Center for Blood and Cancer Disorders (CCBD) department. Identifies needs of the patient population and plans and implements programs to meet those needs. Provides direct clinical services to patients and their families. Coordinates the multi-disciplinary team across the CCBD department of psychosocial care providers through program design, organization of the workload, supervision of social work staff, monitoring of services provided by team members, and program evaluation. Qualifications
Minimum Education:
Master’s Degree in Social Work or equivalent (Required) Minimum Work Experience:
8 years post‑master’s experience as a social worker (Required); 5 years as a social work supervisor/manager in a complex setting (Preferred); 3 years recent experience in a health care setting (Required) Licenses and Certifications:
Ind. Clinical Social Worker (Required) Required Skills/Knowledge:
Excellent oral and written communication skills; Ability to work effectively with persons of diverse cultural backgrounds; Conflict management and problem‑solving skills in complex and/or emotionally charged situations; Understanding of psychosocial issues related to chronic illness, life‑threatening illness, grief and bereavement, and the challenges of the current healthcare environment; Expertise in working with patients and families receiving treatment for oncologic and hematologic diseases. Responsibilities
Develop a comprehensive program of psychosocial services, including definition and structure for support services, integration with primary medical and nursing services, promotion of collaboration among psychosocial care providers, design of tools for assessment and documentation, coordination of the workload of psychosocial team members, and research of patient and family unmet needs. Provide clinical services to children and their families, including comprehensive psychosocial assessment, intervention planning, use of communication skills to address care needs, linkage to appropriate resources in the hospital and community, effective intervention in highly emotional or complex situations, follow‑up services, assessment of intervention outcomes, and documentation in the medical record. Ensure the quality of psychosocial services by convening meetings for case review and program evaluation, supervising social work staff and students, coordinating care with non‑social work members of the psychosocial team, monitoring services provided, designing tools to assess effectiveness and patient/family satisfaction, and supporting the work of Case Managers and Clinical Resource Management initiatives. Collaborate with internal and external customers by participating in medical rounds and case conferences, building effective working relationships with other disciplines and departments, facilitating resolution of customer service issues, supporting the work of Case Managers, providing formal and informal presentations about psychosocial issues, organizing family members and friends for an advisory group, and serving as a liaison to community organizations involved with this population. Deliver organizational accountabilities such as setting and communicating team goals and priorities, developing and assigning resources to meet team goals, providing resources and guidance for employees, developing procedures to ensure high safety and quality, identifying customers’ needs and ensuring service excellence, linking the department and the team, providing feedback to staff, ensuring team adherence to regulations, managing the working environment to promote productivity and motivation, clearing obstacles to high performance, hiring staff and developing their capabilities, and monitoring and promoting strong employee engagement.
