Part time Registered Nurse Case Manager Anticipated Territory : San Mateo County Schedule: Saturday/Sunday What you’ll do: Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

Conduct Start of Care (SOC) Admissions - OASIS

Conduct follow ups, evaluation visits as needed in designated territory

Make home visits to patients in designated territory

Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures

Monitor clients’ conditions, reporting changes to Clinical team

Collaborate with physicians and agency staff to deliver the best care possible

Initiate and Administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols.

Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.

To be a success:

Current and unrestricted CA RN license

Current BLS/CPR Certification

Valid CA Driver's License

Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired

High level of clinical knowledge in home health care is highly preferred

Experience with high acuity patients is a plus

Experience working with an interdisciplinary team is highly desired

Case Management fundamentals is highly desired

Previous experience with IV Medication infusions highly preferred

Proficiencies initiating intravenous (IV) therapy is a plus
