What You’ll Do As an RN Care Manager, you will coordinate services for patients with CKD and ESRD, identify and resolve risks, communicate with care teams, and ensure seamless transitions.

The role is mostly tele‑based with required on‑site visits to healthcare settings or patient homes as needed.

The Day to Day

Collaborate with the care team to create personalized, comprehensive care plans.

Conduct on‑site and virtual assessments to identify undisclosed medical or social issues.

Initiate proactive outreach to patients and clinics per program guidelines.

Manage a diverse patient panel, evaluating needs while adhering to NCQA standards.

Communicate updates, identify gaps, and provide patient education.

Serve as liaison with dialysis facilities and maintain accurate program records.

Analyze Strive data platforms to identify trends and focus areas.

Provide culturally competent care, fostering collaboration among team members.

Apply clinical judgment, analytical skills, and attendance reliability to deliver high‑quality care.

Perform in‑person patient care activities that may involve standing, sitting, walking, and lifting.

Minimum Qualifications

Associate’s degree from an accredited nursing school.

Current Registered Nurse (RN) license in the state of practice.

3+ years of related education, experience, or certification.

Current BLS or CPR certification.

Reliable transportation and active driver’s license for travel across an assigned region.

Internet connectivity: minimum 3.8 Mbps/3.0 Mbps (up/down) with low latency.

Preferred Qualifications

3+ years experience in nephrology, ER, ICU, cardiac, or ambulatory complex case‑management nursing.

Certified Case Manager (CCM) certification.

Proficiency with Microsoft 365 Suite (Word, Outlook, Excel) for a hybrid‑remote environment.

About You

Strong patient and family relationship building skills that promote engagement and positive outcomes.

Experience proactively contacting patients or clinics on a frequent basis to assess needs.

Proficiency in documenting and navigating electronic medical records (EMR) and care‑plan management.

Annual Salary Range $74,000.00–$90,000.00
