Job Overview/Summary We are seeking a compassionate and skilled Licensed Master Social Worker (LMSW) to provide psychosocial assessments, crisis intervention, care coordination, and resource support for patients and families within a hospital setting. This role is central to ensuring safe transitions of care and addressing social determinants of health impacting patient outcomes.
Responsibilities
Conduct psychosocial assessments for patients and families and develop treatment plans.
Interview patients and family members to evaluate needs related to health, social determinants, and resource requirements.
Participate in multidisciplinary rounds, offering psychosocial input.
Serve as a resource and advocate for advanced care planning discussions.
Provide support to patients facing challenging life transitions or crises.
Coordinate discharge planning and post‑hospital care services, including:
Long‑term and custodial care
Acute rehab, sub‑acute/SNF
LTAC, dialysis, hospice
Homelessness support and community resources
Document all assessments and interventions per hospital, state, and Joint Commission standards.
Maintain relationships with community agencies to support patient transitions.
Maintain confidentiality and uphold professional standards.
Primary Skills
Strong communication, negotiation, and conflict resolution skills.
Knowledge of healthcare regulatory standards related to social work.
Familiarity with care coordination and community resource programs.
Ability to support patients across diverse medical and psychosocial needs.
Secondary Skills (Good To Have)
Experience working with homelessness programs or community‑based services.
Knowledge of behavioral health resources and crisis management.
Experience with complex discharge planning.
Qualifications
Master’s Degree in Social Work (MSW) from an accredited school.
Medical hospital social work experience strongly preferred.
