Inova Fairfax Hospital is looking for a dedicated Experienced Case Management Program Manager to join the Case Management Team. This role is Full-Time, Day shift Monday to Friday 8:30 a.m. – 5:00 p.m. with weekend and holiday rotation. The position has hybrid capabilities.

Case Management Program Manager Responsibilities
Serves as resource for discharge planners and the multi-disciplinary team through guidance, training, consultation, participation in Multi-Disciplinary Rounds, and management of assigned patients.
Ensures continuity of care by reviewing patient assessments, ensuring appropriate referrals for Social Determinants of Health needs, and identifying at-risk populations using approved screening tools.
Initiates and facilitates referrals to specialists, clinics, home healthcare, hospice, SNF, acute rehab, LTAC, TCM, medical equipment and supplies as indicated.
Communicates routinely with patients, families, and interdisciplinary teams regarding the status of patients' care plans, progress toward treatment goals, and problem solving.
Seeks consultation from appropriate disciplines or departments to expedite care and facilitate discharge.
Communicates with payers to ensure reimbursement certification for assigned patients and resolves payer issues.
Works with Discharge Planners and care teams to coordinate all areas of care and collaborates on discharge planning needs.
Ensures safe care by adhering to policies, procedures, standards, within budget, and ensuring productivity and accuracy of practice.
Collects and reports resource and financial indicators, including clinical metrics, case mix, LOS, cost per case, readmission rates, denials, and appeals.
Serves on agency committees, work groups, and other bodies.
Responsible for quality assurance in the unit's service delivery, collecting and addressing variances from plans of care.
Performs additional duties as assigned.
Minimum Requirements
Education: Bachelor's Degree in Nursing or Master's Degree in Social Work.
Experience: Minimum of five (5) years acute care case management experience in an acute healthcare environment; demonstrated understanding of discharge planning for specific disease states and Social Determinants of Health.
License: Licensed in the Commonwealth of Virginia as a Registered Nurse (RN) or Social Worker (LCSW).
Certification: Basic Life Support (BLS) for Healthcare Provider from the American Heart Association required upon start. Must also have either Accredited Case Manager (ACM) or Certified Case Manager (CCM) upon start.
Preferred Qualifications
Five (5) or more years of prior inpatient case management experience, discharge planning, and supervisory/lead experience.
Experience working through medically complex cases.
