as a Cardiology Nurse Practitioner or Physician Assistant. In this role, you will provide comprehensive care to patients with heart-related conditions, working closely with cardiologists and a dedicated healthcare team.

Schedule:
Full-Time (5 days per week)
Salary:
$150,000.00 - $200,000.00
Job Summary We are seeking a friendly and compassionate professional to join our team. The successful candidate will play a crucial role in delivering high-quality, proactive care to residents in skilled nursing and long‑term care facilities.
Responsibilities
Perform comprehensive patient assessments, including physical examinations, medical histories, and diagnostic tests.
Collaborate with cardiologists to develop and implement treatment plans for patients with cardiovascular conditions.
Administer medications, monitor patient responses, and adjust treatment plans as necessary.
Conduct cardiac stress tests, electrocardiograms, and other diagnostic procedures.
Educate patients and their families about cardiovascular health, preventive measures, and medication adherence.
Provide counseling and emotional support to patients and families.
Apply knowledge of CMS regulations and NCQA HEDIS guidelines.
Document accurate medical records to ensure compliance with OSHA and governmental regulations, and perform duties in accordance with site quality and safety systems.
Collaborate with the interdisciplinary team.
Analyze and organize the care of critically complex patients.
Requirements
Minimum 6 months of experience in long‑term care or a skilled nursing facility.
Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
DEA License.
CPR BCLS/BLS certified.
Must hold a Master's Degree in Nursing.
Completed a degree from an accredited Nurse Practitioner program.
Preferred experience in cardiology: training provided.
Preferred experience documenting EMR (Sigma, Visual, PCC, etc.): training provided.
Strong organizational and time‑management skills.
Must be able to lift a minimum of 20 pounds and stand or walk on your feet for 6‑8 hours per day.
