Description

Comunilifeis looking for anHRSN Care Coordinatorfor its newSocial Care Network (SCN) Program. Reporting to the Director of SCN Services, the HRSN Care Coordinator manages the patient’s care plan, ensuring they receive appropriate services and support to achieve optimal health outcomes. They act as a liaison between patients, their families, and healthcare providers, coordinating appointments, managing resources, and advocating for the patients' needs. The Coordinator will be responsible for identifying individuals within Comunilife programs and those within the communities served who have Medicaid, ensuring those individuals (members) are screened for eligibility and navigated to the appropriate health- related social services. They will be responsible for coordinating with Comunilife Inc program staff to ensure all members’ needs are met and the support is documented in the SCN data portals, depending on the borough of residence. This position is eligible for Comunilife's generous benefits and paid time off package.

Schedule:

Mon.-Fri. 9am - 5pm

Essential Duties:

Developing and implementing individualized care plans based on patient assessments and needs.

Arranging appointments, facilitating communication between different healthcare providers, and ensuring seamless transitions between care settings.

Connecting patients with relevant community resources, such as social services, support groups, and financial assistance programs.

Representing the patient's interests and ensuring their voice is heard throughout the care process.

Tracking patient progress, evaluating the effectiveness of interventions, and making adjustments to the care plan as needed.

Maintaining accurate and up-to-date records of patient interactions, assessments, and care plan updates.

Working closely with other members of the healthcare team, including physicians, nurses, social workers, and therapists.

Qualifications

Education and Experience Requirement(s)

Bachelor’s Degree in human services or related field; or, Associates Degree with 4 years related experience; or, High School Diploma with 6 years of related case management experience.

Two (2) years’ related work experience in the health care setting.

Experience serving individuals living with HIV/AIDS, chronic mental illness, substance abuse, and

their families is a plus

Bilingual skills in English/Spanish a plus

Knowledge and hands-on experience in providing direct services related to the treatment and recovery of persons with HIV/AIDS, substance abuse, serious mental illness, and homelessness

Excellent oral and written communication skills; proficiency with MS Office and Google product suites

Ability to perform and manage in a team approach; excellent interpersonal skills

Ability to handle complex challenges and balance multiple demands on time and attention

Ability and willingness to perform all duties as workload necessitates
